A: You only need a doctor's note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness. SC1 forms for the self-employed can be obtained from the surgery or from the DSS. Q: How much do you charge for Non NHS services?A: The NHS does not pay for some of the services we provide. These include:
Our fees for these services, which are in line with BMA approved national guidelines, are available in the surgery. Q: Who has access to medical records? A: The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient's representative or outside body will be dealt with in accordance with the Act. Q: Can I get hospital transport and how do I book it? A: Patient transport services are provided to help patients get to their appointment if they have a clinical condition that means they can't get there safely on their own. This is a separate service from the emergency ambulances.
