Q: How do I make a complaint?

A: We take complaints very seriously. If you would like to make a complaint regarding the surgery or the services we offer, please contact the Practice Manager by telephone or, if you prefer, in writing. Every effort will be made to answer your concerns as soon as possible.

Q: When do I need a Sick Certificate (sick note)?

A: You only need a doctor's note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness. SC1 forms for the self-employed can be obtained from the surgery or from the DSS.

Q: How much do you charge for Non NHS services?

A: The NHS does not pay for some of the services we provide. These include:

  • private sick notes
  • insurance forms
  • holiday cancellation forms
  • medical reports
  • fitness to travel certificates
  • private prescriptions
  • some vaccination services

Our fees for these services, which are in line with BMA approved national guidelines, are available in the surgery.

Q: Who has access to medical records?

A: The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient's representative or outside body will be dealt with in accordance with the Act.

Q: Can I get hospital transport and how do I book it?

A: Patient transport services are provided to help patients get to their appointment if they have a clinical condition that means they can't get there safely on their own. This is a separate service from the emergency ambulances.

You are entitled to hospital transport if you:

  • Need a stretcher for the journey or are confined to a wheelchair
  • Need oxygen or other medical gases during the journey
  • Have a mental health problem, learning disability, speech, sight, hearing difficulty which stops you using public transport
  • Cannot walk without relying on another person or a walking aid like a Zimmer frame
  • Have side effects from your medical treatment or condition which stops you using public transport.

If none of these apply to you, you will need to make your own arrangements to get to and from your hospital appointment.

If you are eligible to book transport please telephone the Contact Centre on 0870 600 0668. The centre is open 8.00am to 6.00pm Monday to Friday (it is not open on public holidays).
