Nursing Team

Numerous changes and developments in the role of the Nursing Team are happening within the practice with extensive training to create more flexibility for patients to access health care professionals.

Nurses and Health Care Assistants work in conjunction with the medical profession to facilitate investigations and monitoring of acute and chronic conditions. This role is continually under review with further scope for development planned.

The following is a brief picture of some of the roles and responsibilities undertaken by the nursing team.

Health Care Assistants

We currently have 2 HCAs available at both sites most mornings of the week, who undertake a support role for the nurses and doctors to speed-up the investigation process.

Phlebotomist

Blood taking is done by our phlebotomist.

Nursing Speciality Clinics

Clinic Description
Cryo
Application of liquid nitrogen to ‘freeze’ viral warts/verrucaes and other skin lesions. Appointments are available at Woolpack House Surgery only.
Minor Surgery
The GPs involved in minor surgery will offer an invitation to attend for the excision of moles, skin tags and other procedures under a local anaesthetic. This is a designated session: available at Woolpack House Surgery on Wednesdays.
Diabetic Monitoring
The HCA staff will invite you to attend for baseline observations and blood testing. The results and actions necessary to maximise control of this condition will be discussed with you at a ‘nurse review’ session: Wed pm or Thurs pm. It is essential to attend both appointments.
Healthy Heart Review
The results and actions for management of a range of conditions under 'Healthy Heart' umbrella will begin with a monitoring process by our HCA's. The nursing team will then complete the management and lifestyle sections which are part of the annual review. It is important you attend both appointments - we offer a flexible follow up system.
Respiratory Diseases (Asthma/COPD)
For patients using regular inhaler therapy we would urge you to attend at least annually for a review of your condition. Please bring your inhalers with you when you attend. Appointments can be made to suit your needs via reception.
Vaccinations
Both sites offer flexible access to the nursing team for routine U.K. vaccinations.
Travel Health
All the nurses are skilled in the risk assessment for patients travelling over seas for holidays, business etc. This is a rapidly growing area for the practice: please contact reception for further details about making an appointment. Click here to view travel information leaflet.
Babies and Children
A ‘baby/children’s’ session is available at South Bank Medical Centre on a Tuesday am. Invitations will be offered for your baby/child to attend. Please return invitation slip to the surgery or bring along to your appointment.
2 months old: Diptheria, tetanus, pertussis, polio and Haemophilus influenza type b (DTaP/IPV/Hib), Pneumococcal (PCV)
3 months old: Diptheria, tetanus, pertussis, polio and haemophilus influenza type b (DTaP/IPV/Hib), Meningitis C (Men C)
4 months old: Diptheria, tetanus, pertussis, polio and Haemophilus influenza type b (DTaP/IPV/Hib), Pneumococcal (PCV), Meningitis C (Men C)
Around 12 months: Haemophilus influenza type b, Meningitis C (Hib/Men C)
Around 13 months: Measles, Mumps and Rubella (MMR), Pneumococcal (PCV)
3 years 4 months to 4 years old: Diptheria, tetanus, pertussis, polio, haemophilus influenza type b (DTaP+IPV+Hib) Measles, Mumps and Rubella (MMR)
13 to 18 years old: Tetanus, diptheria and polio (Td/IPV), MMR if not previously given.
Over 75 checks
We offer check ups for patients over 75 years of age. All patients aged 75 or over who have not been seen in the last 12 months will be invited to attend to see our Practice Nurse.
